Membership consists of an eye health and wellness assessment, precise glasses prescription, and frequent individualized appointments using state-of-the-art technology for 12 months. Priority scheduling, extended face-to-face time, and direct communication with the doctors and staff is included.

Ocular Wellness Exam $95

Thorough eye health and wellness assessment including a precise glasses prescription. Advanced diagnostic tools are used to evaluate the health of the eyes and address issues like dry eyes and cataracts.

Advanced Ocular Wellness Exam $150

Thorough eye health and wellness assessment including a precise glasses prescription and specifically tailored for patients with a family history of eye disease or an existing eye condition. State-of-the-art scanning technologies assist in the early detection and monitoring of glaucoma, macular degeneration, corneal diseases, and other vision threatening conditions.

Soft Contact Lens Wellness Exam $195

All elements of the Ocular Wellness Exam plus the additional testing, evaluation, and measurements required to finalize a prescription for soft contact lenses. Includes any diagnostic lenses used and follow-up visits up to 30 days after the initial visit.

Specialty Contact Lens Exam

Problem Focused Office Visit $80

Diagnosis and treatment for an acute or chronic eye condition. Includes eye emergencies, injuries, infections, and ocular disease management.

Follow Up Visit $45

Doctor directed return visits as needed for any ongoing medical eye care treatment or management. Includes any contact lens related follow-up visit outside of the initial 30 days from examination.
